Diploma Application Form for Students
in the Graduate School of Engineering Doctoral Degree Program

Students who complete the Doctoral Degree Program will receive diplomas in both Japanese and English.
Students are requested to submit a “Diploma Application Form for Students in the Graduate School of
Engineering Doctoral Degree Program.” Please click the link below to download the application form.
Before filling out the form, please read carefully the notes below.

Notes:
-Please write your name in block letters as shown on your passport.
– If you wish to have your name written in Chinese letters on your Japanese diploma,
　please write your name in Chinese letters as well.
– If you are still undecided on which title of degree to choose, you can leave the boxes unchecked.
　(In this case, the title stated on the Degree Review Report submitted by your chief examiner will be
　shown on your diploma.) Please note that the title of the degree on a Japanese diploma must
　correspond with the title of degree on an English diploma.
-Your field will come after “IN THE FIELD OF” on your diploma. For your field, please indicate
　the name of your department. (See the Diploma Application Form.)
– After filling out the form, you are requested to ask your academic supervisor to approve the content
　by signing and sealing the form.
– Diplomas cannot be reissued. Please check carefully what you have written on the form before submission.
